Job Summary

A company is looking for a Bilingual Telephonic RN Case Manager, Maternal Health.

Key Responsibilities

• Engage members telephonically to conduct comprehensive needs assessments
• Develop and implement person-centered care plans addressing various health needs
• Collaborate with care teams and community resources to implement care plans

Required Qualifications

• Current, unrestricted WA state or multi-state RN licensure
• 2+ years of Maternal clinical experience
• Bilingual proficiency in English and Spanish (verbal and written)
• 1+ years of experience with MS Office applications
• Designated quiet workspace with secure high-speed internet access
