Washington Licensed Care Coordinator
Job Summary
To support patient care management, the full-time Washington Licensed Care Coordinator will manage a panel of 400-450 patients, conduct monthly health coaching outreach, and collaborate with clinical teams, all while working remotely in a HIPAA-compliant home office.
Key responsibilities
- Conduct 50-60 daily outbound calls to provide care plan support and health coaching
- Collaborate with clinical teams and caregivers to address patient clinical and social needs
- Document all patient interactions in real time while maintaining HIPAA compliance
Required qualifications
- Must hold a nationally recognized medical assistant certification or be a Certified Pharmacy Technician
- Minimum 2 years of clinical experience in a healthcare setting
- Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
- HIPAA-compliant home office setup with minimum internet speed requirements
- Must be legally authorized to work in the United States
