Washington Licensed Case Manager

Job Summary

To support a growing team, the full-time Washington Licensed Case Manager will engage with members facing medical, mental health, and substance use disorders, conducting holistic assessments and developing personalized case management plans while working remotely.

Key responsibilities

• Engage with target members to build supportive relationships and facilitate open discussions on sensitive health topics
• Conduct initial holistic health assessments to identify member needs and social determinants of health
• Create and monitor member-centric case management plans, coordinating care among healthcare providers

Required qualifications

• Active WA State or Compact License in nursing, behavioral health, or psychology
• 3 years of experience in direct patient or clinical care post-licensure
• Internet access with a minimum speed of 75 Mbps
