Job Summary
Working remotely as a full-time Washington Licensed Case Manager, the professional will engage with members facing mental health and substance use disorders, conduct holistic health assessments, and develop collaborative case management plans to improve health outcomes.
Key responsibilities:
- Engage members in constructive dialogue regarding mental health and substance use challenges
- Conduct initial holistic health assessments to identify specific needs and barriers
- Create and monitor member-centric case management plans in collaboration with caregivers and providers
Required qualifications:
- Active WA State or Compact license for independent assessment (e.g., LMFT, LICSW, LMHC, LPC)
- 3 years of post-licensure direct care experience
- 1 year of post-licensure case management experience
