Job Summary
A company is looking for a Clinical Pharmacy Specialist who will elevate patient care through health-system integrated specialty pharmacy.
Key Responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, and adherence support
- Collaborate with physicians and interdisciplinary teams to create individualized care plans under a Collaborative Practice Agreement (CPA)
- Monitor therapy effectiveness and safety, and support quality improvement initiatives
Required Qualifications, Training and Education
- Doctor of Pharmacy (PharmD) degree
- Licensed pharmacist in Washington state
- Completion of a PGY2 Ambulatory Care Residency or a PGY1 Residency with at least 1 year of ambulatory practice under a CPA
- At least 3 years of ambulatory practice with disease state management
- Strong organizational and technical skills
