Job Summary

A company is looking for a NICU Case Manager, RN - Remote in WA.

Key Responsibilities

• Engage members to complete comprehensive needs assessments covering medical, behavioral, and socioeconomic needs
• Develop and implement person-centered care plans to manage chronic health conditions and promote wellness
• Collaborate with internal care teams and community resources to implement care plans and support member self-management

Required Qualifications

• Current, unrestricted RN licensure in the state of Washington
• 2+ years of NICU clinical experience
• 1+ year of experience with MS Office applications
• Reside in the state of Washington
• Designated workspace and secure high-speed internet access
