Washington Licensed Nurse Practitioner

Job Summary

To support a growing care practice, the part-time contract Washington Licensed Nurse Practitioner will provide high-quality virtual care services to senior patients living with chronic conditions, collaborating with a transdisciplinary team to develop and implement treatment plans and ensure comprehensive care.

Key responsibilities

• Navigate and manage complex chronic comorbidities for patients and their families
• Create and implement personalized treatment plans in collaboration with the healthcare team
• Monitor patient progress and adjust care plans as necessary while providing clinical education

Required qualifications

• Two or more years of outpatient experience in independent practice or a larger medical center
• Washington state Nurse Practitioner and Registered Nurse licenses required
• Board certification as a Nurse Practitioner in geriatric care preferred
• Master's level ARNP required, with additional training or experience in Geriatric care, Dementia, and Palliative specialties
• Experience utilizing chronic, longitudinal care models in digital health required
