Job Summary
To enhance patient care at a nationally recognized Academic Medical Center, the part-time Washington Licensed Oncology Pharmacy Specialist will provide comprehensive medication management, collaborate with care teams under a Collaborative Practice Agreement, and conduct patient assessments both in-person and virtually.
Key responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, and adherence support
- Partner with physicians and interdisciplinary teams to order individualized care plans and promote medication coordination
- Monitor therapy effectiveness and safety, identifying optimization opportunities and intervening as necessary
Required qualifications
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in Washington
- Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with at least 3 years of clinical oncology pharmacist experience
- At least 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a Collaborative Practice Agreement (CPA)
- Strong communication, organization, and technology skills
