Job Summary

A company is looking for a Pediatric / NICU Case Manager, RN - Remote Living in WA.

Key Responsibilities

• Engage members to complete comprehensive needs assessments addressing medical, behavioral, and socioeconomic needs
• Develop and implement person-centered care plans in alignment with evidence-based guidelines
• Collaborate with care teams and community resources to support member self-management and coordinate care transitions

Required Qualifications

• Current, unrestricted RN licensure in the state of Washington
• 2+ years of NICU and/or Pediatric clinical experience
• 1+ year of experience with MS Office applications
• Reside in the state of Washington
• Designated workspace with secure high-speed internet access
