Washington Licensed Pediatric Pharmacist

Job Summary

To support a growing pediatric practice, the remote Pediatric Clinical Pharmacist will provide medication management services for pediatric patients with chronic medical and behavioral health conditions, optimizing medication therapy and collaborating with an interdisciplinary care team while working Monday to Friday from 8:00am to 5:00pm PST.

Key responsibilities

• Provide comprehensive medication management services, including access, education, adherence, and quality metric support
• Assist with medication access management between patients, pharmacies, insurance, and medical providers
• Document patient encounters and promote evidence-based medication management practices

Required qualifications

• Graduate of an accredited college/university of pharmacy (Pharm.D) required
• Unrestricted, current pharmacist license with the ability to obtain licensure by reciprocity in other states; active Washington license preferred
• Completion of ASHP accredited PGY-1 Pharmacy Residency; PGY-2 residency preferred
• 2+ years of practice experience in pediatrics, preferably in a pediatric or ambulatory care setting
• Bilingual in English/Spanish preferred
