Job Summary

A company is looking for a Population Health Care Manager (Hybrid w/ Possibility 100% Remote).

Key Responsibilities:

• Engage patients proactively with various forms of communication
• Achieve and maintain high levels of engagement with patient caseload, consistently meeting monthly engagement targets
• Effectively communicate and collaborate with providers and patient care team to ensure seamless care delivery

Required Qualifications, Training, and Education:

• Minimum of 2 years in healthcare or patient care management
• Current State of Washington Medical Assistant - Certified or LPN license
• Knowledge of healthcare systems and patient engagement strategies
• Familiarity with healthcare management software and data systems
• Strong organizational skills to manage large caseloads
