Washington Licensed RN Case Manager

Job Summary

A company is looking for a Bilingual Case Manager, RN - Maternal Health.

Key Responsibilities:

• Engage members to complete comprehensive needs assessments including medical, behavioral, and socioeconomic factors
• Develop and implement person-centered care plans addressing chronic health conditions and promoting wellness
• Collaborate with care teams and community resources to support member self-management and care transitions

Required Qualifications:

• Current, unrestricted RN licensure in the state of Washington
• 2+ years of Maternal clinical experience
• 1+ year of experience with MS Office applications
• Bilingual in English and Spanish
• Designated quiet workspace with secure high-speed internet access
