Washington Licensed RN Case Manager

Job Summary

To support kidney health management, the remote Washington Licensed RN Case Manager will manage a caseload of members, facilitating care coordination and promoting high-quality outcomes through education, advocacy, and collaboration with healthcare teams.

Key responsibilities

• Handle inbound and outbound calls, providing exceptional service to members
• Educate members on kidney health, dialysis, and transplant options while engaging them in HealthMap's Kidney Health Program
• Collaborate with interdisciplinary care teams to coordinate care and resolve patient barriers

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience, including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
