Job Summary
Washington Oncology Clinical Pharmacy Specialist is a remote, full-time position responsible for providing comprehensive medication management and collaborating with care teams to enhance patient care in oncology.
Key Responsibilities
- Provide comprehensive medication management, including dose optimization and adherence support
- Collaborate with physicians and interdisciplinary teams to create individualized care plans
- Monitor therapy effectiveness and safety, and support quality improvement initiatives
Required Qualifications, Training, and Education
- Doctor of Pharmacy (PharmD) degree
- Licensed pharmacist in Washington
- Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency with 3 years of clinical oncology experience
- At least 3 years of experience as a clinical pharmacist in oncology, including 1 year under a Collaborative Practice Agreement
- Strong organizational and technical skills
