Job Summary

A company is looking for a Pediatric Nurse Care Manager to provide holistic care and support to members through their healthcare journey.

Key Responsibilities:

• Deliver coordinated, patient-centered virtual Care Management via telephone and video
• Create impactful care plans with members and the care team to achieve health goals
• Assist members in navigating complex medical conditions and the healthcare system

Required Qualifications:

• Bachelor of Science in Nursing (BSN)
• Current CCM Certification or eligibility within 6 months of hire
• 5+ years of nursing experience, with 2+ years in care management
• Active Compact RN license in good standing
• Must reside in a compact NLC state and be willing to maintain licensure in multiple states
