Washington RN Care Manager
Job Summary
A company is looking for a Care Manager (RN).
Key Responsibilities
- Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
- Develop ongoing care plans and collaborate with providers to meet members' unmet needs
- Provide education to members and their families on healthcare options and monitor progress towards care goals
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
