Washington RN Case Manager

Job Summary

A company is looking for an RN Case Manager.

Key Responsibilities

• Ensure that patients and families experience medically appropriate transitions of care
• Advocate for patients and families, facilitating transitions to lower levels of care as needed
• Partner with physicians and advanced practice providers in patient care activities and care transitions

Required Qualifications

• Bachelor's degree in Nursing or obtained within five years from the initial date of hire
• Current Registered Nurse license in Washington State or Multistate License endorsement
• Four years of recent hospital case management experience
• Five years of recent clinical hospital or ambulatory experience
• Advanced degree in Nursing preferred
