Washington RN Case Manager

Job Summary

A company is looking for an RN Case Manager for the IKC National Flex Team, working remotely.

Key Responsibilities

• Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
• Collaborate with healthcare team members to minimize fragmentation in care delivery
• Manage care coordination between patients, families, physicians, and care teams

Required Qualifications, Training, and Education

• A.D.N degree from an accredited school of nursing required; B.S.N preferred
• Minimum of 1 year of experience as an IKC RN Case Manager required
• Minimum of 5 years' experience in clinical nursing required
• Current CPR certification required
• Compact Nursing License highly preferred
