Washington RN Case Manager

Job Summary

Covering various IKC markets remotely, the full-time salaried Washington RN Case Manager will coordinate and facilitate quality care for ESRD and CKD patients while collaborating with healthcare teams to ensure effective care delivery.

Key responsibilities

• Manage care coordination between patients, families, physicians, and care teams to minimize healthcare fragmentation
• Function autonomously while collaborating with the healthcare team to deliver cost-effective care
• Utilize data to manage clinical, financial, and patient satisfaction outcomes effectively

Required qualifications

• A.D.N degree from an accredited nursing school required; B.S.N preferred
• Minimum of 1 year of experience as a DaVita IKC RN Case Manager required
• Compact Nursing License preferred
• Minimum of five (5) years' experience in clinical nursing required
• Current CPR certification required
