Washington RN Case Manager

Job Summary

To support a growing healthcare team, the full-time Washington RN Case Manager will coordinate and facilitate quality care for ESRD and CKD patients remotely, collaborating with healthcare teams and managing patient care transitions.

Key responsibilities

• Manage care coordination between patients, families, and healthcare providers to ensure effective treatment plans
• Collaborate with team members to minimize fragmentation in the healthcare delivery system
• Utilize clinical data to monitor and improve patient outcomes and satisfaction

Required qualifications

• A.D.N degree from an accredited nursing school; B.S.N preferred
• Current Registered Nurse (RN) license in Washington state
• Minimum of five years of clinical nursing experience required
• Experience in renal nursing preferred, with a focus on managing complex patient needs
• Current CPR certification and ability to adapt to electronic processes required
