Job Summary

A company is looking for a Clinical Program Coordinator RN - Remote.

Key Responsibilities

• Provide care coordination services, including disease management and case management for members with complex health needs
• Assist members and their families in managing health plan benefits and finding alternative services when needed
• Work to improve access and quality of care while managing medical expenses within the health plan framework

Required Qualifications

• 5 years of clinical nursing experience
• Current unencumbered Registered Nurse License in state of residency upon hire
• Experience collaborating with physicians in patient care management
