Job Summary

A company is looking for a Clinical Program Coordinator RN - Case and Disease Program - Remote.

Key Responsibilities

• Provide care coordination services to members, including disease management and case management
• Educate and empower members to manage their health conditions
• Assist with triage, referral, transition of care planning, and end-of-life care planning

Required Qualifications

• Current unencumbered Registered Nurse License in state of residency upon hire
• 5 years of clinical nursing experience
• Experience collaborating with physicians in patient care management
