Washington RN Maternal Case Manager

Job Summary

To support Medicaid members who are pregnant or recently pregnant, the remote Washington RN Maternal Case Manager will engage with clients to conduct comprehensive needs assessments, develop person-centered care plans, and coordinate care while advocating for members' health needs.

Key responsibilities:

• Engage members telephonically to complete comprehensive needs assessments covering medical, behavioral, and socioeconomic factors
• Develop and implement care plans addressing chronic health conditions and promote wellness in alignment with evidence-based guidelines
• Collaborate with internal care teams and community resources to ensure effective implementation of care plans and support member education

Required qualifications:

• Current, unrestricted RN licensure in the state of Washington
• 2+ years of maternal clinical experience
• 1+ years of experience with MS Office applications, including Word, Excel, Outlook, and Teams
• Designated quiet workspace and access to secure high-speed internet via cable/DSL
• Residency in the state of Washington
