Washington RN Referral Coordinator

Job Summary

To support efficient patient transitions, the full-time Bed Readiness RN Referral Coordinator will manage and coordinate patient referrals across post-acute settings, serving as a liaison between hospitals and external partners while working remotely.

Key responsibilities:

• Coordinate and manage all incoming and outgoing referrals for post-acute placement, ensuring accuracy and timely processing
• Assess patient clinical needs and collaborate with interdisciplinary teams to determine appropriate care and placement options
• Monitor bed availability and placement timelines to optimize patient flow and reduce discharge delays

Required qualifications:

• Bachelor's degree in Nursing, or obtained within five years from the initial date of hire
• Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
• Current BLS Certification required
• ACLS, NALS, PALS, TNCC, or NRP certification as required by facility or department
• One year of experience in an acute care setting preferred
