Job Summary

A company is looking for a Care Manager Transition of Care - Discharge.

Key Responsibilities

Evaluate member needs and complete post-discharge assessments for those transitioning from healthcare facilities
Develop care/service plans and collaborate with various healthcare teams to support member transitions
Facilitate education and resources for members and caregivers to promote overall wellness and improve care quality

Required Qualifications

Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
2 - 4 years of related experience in care management
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
Residency in Washington State is required for this remote position
