Job Summary
Passionate about mental health, the full-time Washington State Care Manager for the Suicide Prevention Program will provide compassionate support, engage with members, and coordinate care remotely while working Monday through Friday from 10:00 AM to 6:30 PM Pacific Standard Time.
Key responsibilities:
- Assist with outreach and continuous support for members, providing regular check-ins and personalized care
- Develop and coordinate community resources to address medical, behavioral, and social service needs
- Assess risk and perform crisis interventions to ensure safety through tailored safety plans
Required qualifications:
- Bachelor's degree in psychology, social work, or a related behavioral health discipline
- At least 2 years of experience in a related role, preferably with at-risk adolescents and young adults
- Crisis intervention training or similar certification is required
- Knowledge of local resources and social determinants of health
- Understanding of mental health, substance abuse disorders, and interpersonal violence issues
