Washington State Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans in collaboration with healthcare teams to support member transitions
• Facilitate communication and services among medical and non-medical resources to meet member needs

Required Qualifications, Training, and Education

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
• 2-4 years of related experience
• Bilingual (Spanish) highly preferred
• WA State RN License required
• Relevant licensure (LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN) required
