Job Summary
To support quality healthcare outcomes, the full-time remote Senior Care Manager (RN) will assess, plan, and implement complex care management activities while developing personalized care plans for members and educating families on available services.
Key responsibilities:
- Develop and continuously assess ongoing care plans, collaborating with providers to address members' unmet needs
- Coordinate care between members, families, and care provider teams to ensure person-centered services
- Monitor care plans and member status, revising as necessary to meet goals and improve quality care
Required qualifications:
- Degree from an accredited school of nursing or a Bachelor's degree in Nursing
- 4-6 years of related experience in care management
- WA State RN Licensure and/or Compact State Licensure required
- Experience in developing and implementing care plans
- Ability to collaborate effectively with healthcare providers and community resources
