Washington State Licensed Care Manager
Job Summary
To support high-quality, cost-effective healthcare outcomes, the full-time remote Washington State Licensed Care Manager will develop and facilitate complex care management activities, focusing on personalized care plans and education for members and their families.
Key responsibilities
- Evaluate member needs and barriers to care, recommending and facilitating plans for optimal outcomes
- Develop ongoing care plans and collaborate with providers to address members' unmet needs
- Coordinate care between members, families, and providers to ensure person-centered services
Required qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in care management
- Washington State RN Licensure and/or Compact State Licensure required
- Experience working with maternal and infant populations preferred
- Familiarity with supporting members in behavioral health crises is strongly preferred
