Job Summary

A company is looking for a Case Manager.

Key Responsibilities

• Engage target members facing medical, mental health, and substance use disorders to support open dialogue
• Conduct holistic health assessments to identify member needs and social determinants of health
• Develop and monitor member-centric case management plans to improve health outcomes

Required Qualifications

• Current Registered Nurse (RN) license allowing independent assessment
• Three years of experience in direct patient or clinical care post-licensure
• High-speed internet connection of at least 20 Mbps
