Washington State Licensed Case Manager

Job Summary

A company is looking for a Remote / Telephonic Case Manager - Bilingual Spanish / English.

Key Responsibilities:

• Deliver high-quality care through proactive communication with members in their preferred language
• Assist members with safe transitions from facilities back to home environments
• Coordinate follow-up appointments and services to ensure continuity of care

Required Qualifications:

• Current, unrestricted WA state (or multi-state) RN license
• 2+ years of healthcare experience
• 1+ years of experience with complex, high-risk, or chronic condition populations
• Bilingual proficiency in English and Spanish (verbal and written)
• Access to high-speed internet from home
