Washington State Licensed Case Manager

Job Summary

Providing holistic primary care management, the full-time salaried Washington State Licensed Case Manager will monitor member utilization, collaborate with external entities, and manage complex care for clients in Bellevue, WA.

Key responsibilities

• Demonstrate nursing skills in assessment, planning, implementation, coordination, monitoring, and evaluation of treatment plans
• Provide ongoing review management of treatment progress to ensure personalized support and care coordination for complex medical conditions
• Conduct Utilization Management reviews to evaluate services requested and ensure continuity of care coordination

Required qualifications

• Baccalaureate degree in Nursing or Community/Public Health (preferred)
• 1-3+ years of clinical nursing experience
• Active RN clinical license or certification, as applicable to the degree
• Strong experience in clinical practice with diverse diagnoses
• Knowledge of Case Management and Utilization Review processes
