Job Summary

A company is looking for a Case Manager, RN.

Key Responsibilities

• Engage with target members to build supportive relationships and facilitate open dialogue
• Conduct holistic health assessments to identify member needs and social determinants of health
• Create and monitor member-centric case management plans to improve health outcomes

Required Qualifications

• Active WA State or Compact License in Nursing, Behavioral Health, or Psychology
• 3 years' experience with direct patient/clinical care post-licensure
• High-speed internet connection (minimum 75 Mbps)
