Washington State Licensed Clinical Care Coordinator

Job Summary

Assisting with care management activities, the full-time Washington State Licensed Clinical Care Coordinator will coordinate member services, facilitate care among providers, and document the care management process while working remotely.

Key responsibilities

• Communicates with providers to arrange follow-up appointments and assist members with care coordination
• Documents and manages the care management process, including case identification, assessment, and planning
• Develops member-centric care plans and provides behavioral health support to members in crisis

Required qualifications

• Bachelor's degree in social work, Practical Nursing Certificate, or equivalent health care degree
• Minimum of one year of clinical and/or care management experience
• Proficiency with Microsoft Office products
• Knowledge of Medicare and Medicaid regulations
• Experience in care management within a health plan or community agency (preferred)
