Washington State Licensed Health Coach

Job Summary

A company is looking for a Health Coach to support members with chronic conditions in achieving optimal health.

Key Responsibilities

• Provide disease-specific care management interventions and promote self-management education
• Assess members' needs and coordinate access to appropriate care and treatment
• Document member interactions and develop individualized plans of care in collaboration with healthcare teams

Required Qualifications, Training, and Education

• Bachelor's degree in a relevant field or equivalent experience required
• Current, unrestricted license in Washington as LPN, RN, RD, or other relevant licensure preferred
• Relevant certification such as Health Coach or Certified Diabetes Care and Education Specialist (CDCES) preferred
• Minimum of two years of experience in disease management, diabetes education, or health coaching preferred
• Experience in Managed Care, Medicare, and/or Medicaid preferred
