Washington State Licensed RN Case Manager

Job Summary

Providing comprehensive care management remotely, the Washington State Licensed RN Case Manager will assess, plan, implement, and evaluate individual member needs while ensuring continuity of care and cost-effectiveness in a full-time salaried role.

Key responsibilities:

• Develop and implement individualized care management plans addressing specific member needs
• Monitor member adherence to treatment plans and collaborate with multidisciplinary teams to address issues
• Educate and support members and their families in managing health care services and treatment regimens

Required qualifications:

• Degree in Nursing from an accredited school of nursing
• Unrestricted RN licensure in Washington State
• CCM Certification within three years of employment
• Experience in care management within a health care setting
• Knowledge of health care delivery systems and care management practices
