Job Summary

As a full-time telecommuting Washington State Licensed RN Case Manager, the successful candidate will engage with members facing medical utilization disorders, conduct holistic health assessments, and develop collaborative case management plans to improve health outcomes.

Key responsibilities

• Engage members to build relationships and facilitate open discussions on sensitive health topics
• Conduct initial holistic health assessments to identify specific needs and social determinants of health
• Monitor and update case management plans, ensuring effective communication and coordination among healthcare providers

Required qualifications

• Active Washington State or Compact RN or ARNP license allowing independent assessments
• 3 years of post-licensure direct care experience
• 1 year of post-licensure case management experience involving care coordination or management
