Washington State RN Care Manager

Job Summary

A company is looking for an RN Care Manager.

Key Responsibilities

• Improve health outcomes and reduce care costs for children through care management
• Implement transitional care, ED visit follow-up, and care coordination strategies
• Educate staff and families on care coordination processes and track patient outcomes

Required Qualifications, Training, and Education

• BSN degree or equivalent combination of education and experience
• Minimum of five (5) years of recent clinical experience
• Experience in community-clinic settings and case management for complex children and families
• Current Washington State Nursing License and Drivers License
• Current BLS for Healthcare Providers certification
