Key Responsibilities:
  • Completes telephonic outreach to members and educates them on gaps in care
  • Assists members with social determinants of health and links them to community resources
  • Maintains schedule adherence and ensures compliance with health information regulations
Required Qualifications:
  • High School Diploma / GED or equivalent work experience
  • 1+ years of call center and/or telephonic customer service experience
  • 1+ years of professional experience in an office setting using telephone and computer
  • 1+ years of Healthcare/insurance or community outreach experience
  • Ability to work from home with high-speed internet access
