Job Summary

A company is looking for a Telephonic Care Navigator.

Key Responsibilities
  • Completes telephonic outreach to members and assists with scheduling provider appointments
  • Educates members on gaps in care and links them to community resources
  • Maintains confidentiality of health information and ensures adherence to program guidelines


Required Qualifications
  • High School Diploma / GED or equivalent work experience
  • 1+ years of call center and/or telephonic customer service experience
  • 1+ years of professional experience in an office setting using telephone and computer
  • 1+ years of experience in healthcare/insurance or social work/community outreach
  • Ability to work full-time, Monday - Friday, with flexibility for shift schedules
