Weekend Care Advanced Practice Provider

Job Summary

To support weekend clinical coverage, the full-time Weekend Care Advanced Practice Provider (NP or PA) will conduct virtual visits, manage prescription refills, and collaborate with a cross-functional team, requiring regular Saturday and Sunday work in a fully remote setting.

Key responsibilities

• Conduct thorough assessments and develop individualized treatment plans for patients with chronic conditions
• Manage a prescription refill inbox and participate in centralized results management workflows
• Utilize digital and AI-assisted clinical tools to enhance patient care and improve operational efficiency

Required qualifications

• Active and in-good-standing RN and APRN licenses for Nurse Practitioners or an active Physician Assistant license
• Three or more years of outpatient clinical practice experience, with a focus on chronic disease management
• Minimum of two years of experience in primary care or a subspecialty related to hypertension and lipid management
• Ability to obtain licensure in multiple states within 90 days of hire
• Experience with virtual care and digital health platforms is preferred
