Weekend Intake Coordinator
Job Summary
Covering weekends, the full-time Intake Coordinator will manage home health referrals, ensuring accurate data processing and coordination of care for patients while working remotely.
Key responsibilities
- Process agency home health referrals accurately and in a timely manner
- Complete basic diagnosis coding based on referral information
- Coordinate care for homecare patients using the Welcome Call Script
Required qualifications
- Associate's Degree preferred
- At least one year of experience in Home Health preferred
- Minimum one year of experience in a customer service role
- Experience with Homecare Homebase (HCHB) is required
- Availability to work every Saturday and Sunday from 6:30 AM to 5 PM EST
