A company is looking for a Wellness Associate II to support community-based population management initiatives and wellness programs.

Key Responsibilities

• Develops and designs health and wellness programs in collaboration with assigned facilities
• Implements tools to evaluate program effectiveness and reports outcomes to stakeholders
• Provides on-site support for wellness initiatives and represents the facility at community events

Required Qualifications

• Bachelor's Degree in a healthcare-related field
• Minimum of 1 year of related work experience
• Experience in program development and implementation
• Ability to learn new software applications and maintain databases
• Experience in health coaching and community outreach initiatives
