Wellness Coach

Job Summary

A company is looking for a Wellness Coach (Remote in Northeastern states).

Key Responsibilities

• Connect with members to develop and support health care, addressing barriers and solutions
• Establish supportive relationships with members and caregivers to guide access to resources
• Conduct outreach calls to enroll eligible patients in the Healthmap Kidney Health Management program

Required Qualifications

• Associate's degree in a relevant field or equivalent experience with certification
• 2 years of experience in healthcare, preferably with chronic diseases or behavioral health needs
• Experience in managed care, physician office, or account management preferred
• Familiarity with Medicare and Medicaid preferred
• Previous experience in a metrics-driven environment preferred
