Wellness Coach

Job Summary

To support a growing team, the full-time remote Wellness Coach will interact with members to address care needs, promote wellness, and improve health outcomes through education and assessments while managing a caseload via various communication platforms.

Key responsibilities

• Connect with members to identify and address barriers to healthcare, including scheduling and community resources
• Establish supportive relationships with members and caregivers to facilitate access to necessary resources
• Conduct outreach calls to enroll eligible patients in the Healthmap Kidney Health Management program

Required qualifications

• Associate's degree in a relevant field or equivalent experience with certification
• 2 years of experience in healthcare, preferably with chronic diseases or behavioral health needs
• Experience in managed care or physician office settings preferred
• Familiarity with Medicare and Medicaid preferred
• Previous experience in a metrics-driven environment preferred
