Wellness Coordinator

Job Summary

Supporting underserved teammates, the remote Wellness Coordinator - Louisiana/Texas will provide community support services, assist with resource navigation, and conduct needs assessments, while being based in either Louisiana or Texas.

Key responsibilities

• Conduct and analyze needs assessment data to identify social determinants of health and appropriate community resources
• Utilize Christus Health's LiveWell platform to connect team members with necessary support and resources
• Build and maintain relationships with community partners and communicate with department managers to ensure proper support for team members

Required qualifications

• Bachelor's degree required, preferably in Health Education; MSW or Certified Health Coach preferred
• Two years of experience in healthcare required
• Ability to communicate effectively, both verbally and in writing
• Willingness to be onsite to support all shifts, including occasional weekends and nights
• Strong organizational skills to manage detailed tasks under time constraints
