Job Summary

A company is looking for a Care Transitions Coordinator - Population Health.

Key Responsibilities

• Coordinate and facilitate patient discharge planning during hospitalization and transitions to/from care facilities
• Assess and monitor the plan of care in collaboration with patients and healthcare teams
• Conduct concurrent chart reviews and identify patient needs during transitions of care

Required Qualifications

• Associate's Degree in Nursing with five years of clinical experience or Bachelor's Degree in Nursing with three years of clinical experience
• Current Registered Nurse license in the state of service or multi-state Registered Nurse license through eNLC
• Certification in Basic Life Support within 30 days of hire
• Experience in medical management for Medicare and/or Medicaid populations is preferred
• Prior care coordination experience is preferred
