Job Summary

A company is looking for a Care Manager RN.

Key Responsibilities

• Manage and triage member self-referrals to care management programs
• Assist members in understanding their medical benefits and connecting them with in-network providers
• Review and evaluate Health Risk Assessment data to drive the development of programs and services

Required Qualifications

• Current unencumbered licensure as a Registered Nurse
• Three years of healthcare clinical experience
• Bachelor's Degree in Nursing or currently enrolled in a BSN program (completion within three years preferred)
• Experience managing Medicare and/or Medicaid populations is preferred
• Two years of Care Management experience is preferred
