Job Summary

A company is looking for a Clinical Social Worker - Behavioral Health Integration - Population Health.

Key Responsibilities

• Conducts psychosocial assessments and develops individualized care plans for patients and families
• Collaborates with a multidisciplinary team to provide comprehensive mental health support and monitors patient progress
• Implements psychosocial treatment plans and provides psychoeducation and direct care management

Required Qualifications

• Masters Social Work Degree
• LGSW with 2 years of direct clinical experience in behavioral health
• Preferred LCSW or LICSW certification in the state of West Virginia
• Knowledge of therapeutic techniques and interventions
• Ability to work autonomously and manage multiple assignments efficiently
