Job Summary

A company is looking for a Nurse Navigator-Clinic.

Key Responsibilities

• Triaging incoming patient referrals and assessing needs for appropriate clinical dispositions
• Facilitating referrals and coordinating care across multiple providers and settings
• Providing patient education regarding care advice and preventive behaviors

Required Qualifications

• Current unencumbered licensure as a registered professional nurse
• Three years of registered nursing experience
• Bachelor of Science in Nursing Degree (BSN) preferred
• Certification in Basic Life Support within 30 days of hire preferred
• Experience in care coordination and outpatient/clinic settings preferred
